Plus, FEP Blue Standard™ and FEP Blue Basic™ members can earn $50 the first time they take the BHA each calendar year.*
Here’s how it works:
Sign up for MyBlue
- The BHA is available only through MyBlue, our secure member website. You can sign up now, it takes just a few minutes.
Take the BHA
- Answer a short questionnaire about your health.
- You’ll receive a personalized score and action plan to improve or maintain your score.
Earn $50
- FEP Blue Standard and FEP Blue Basic members earn $50 the first time they take the BHA each calendar year.
- Any rewards earned will be loaded onto your MyBlue Wellness Card. You can use these funds for qualified medical expenses, such as copays and prescriptions.
- FEP Blue Focus™ members can take the BHA to receive a personalized action plan but are not eligible to earn rewards.
Get rewarded for taking an active role in your health
The BHA is the first step in our Wellness Incentive Program. You may also be eligible to earn an additional $120 for completing up to three Daily Habits goals as well as other rewards and incentives for managing serious health conditions.
