Routine Annual Physical Incentive Program: Get Rewarded for Taking Care of Your Health

Why annual physicals matter
Getting an annual physical is an important part of staying healthy. During your visit, your doctor will provide any needed screenings, vaccines and services. As an FEP Blue Focus® member, you can earn rewards for having your routine annual physical.


Visits that qualify
Both routine annual physicals and well-woman visits—where your doctor checks for breast and cervical cancer—qualify for the Routine Annual Physical Incentive Program. You can earn rewards whether you have both a well-woman visit and an annual physical or just a well-woman visit.


How to participate

  • Schedule your visit: See your primary care doctor for an annual physical. If you don’t have a primary care doctor, you can find one by downloading the fepblue app or using our National Doctor and Hospital Finder.
  • Receive your reward notification: After we process the claim from your visit, you’ll get an email or a message on your Explanation of Benefits with instructions on how to claim your reward.
  • Claim your reward: Follow the instructions and log in to your MyBlue® account. If you’re a first-time user, you’ll need to register. Once logged in, select your reward and complete the required fields to receive it.

 

Redeem your reward
MyBlue® Wellness Card: Get $150 on a MyBlue Wellness Card, which you can use with several health and wellness vendors on Blue365® or to pay for qualified medical expenses. You can review participating Blue365 health and wellness vendors here. To see participating Blue365 vendors, log in, accept the terms for Blue365 and select “Wellness Card” under Category to filter the deals you can use with your MyBlue Wellness Card.

Location restrictions apply. You must be 18 or older and the contract holder or spouse on an FEP Blue Focus plan to earn this reward.
